The total number of measures reported on between the "core" and "alternate" measure sets should total three.
Core Quality Measures
If applicable, EPs should report on the following three “core” quality measures.

Alternate Core Quality Measures
If the denominator for one or more of the “core” measures listed above is 0, EPs should default to the “alternate” core measures below and report on one or more of the “alternate” measures.

Additional Quality Measures
In addition to the “core” and/or “alternate” core measures, EPs should choose to report on any 3 from the following list of “additional” measures. If all six of the “core” and “alternate” core measures have a denominator of 0, the EP may choose any three of the “additional” measures and only report on those three measures.
