Medical Claims Overview

UB04 is the standard form used by institutional-based health care providers throughout the U.S. This form is used by hospitals, nursing homes, hospice, home health, and other facility-based providers. It replaced the UB92 claim form in March 2007, to align the paper form to the electronic data standards. These standards are 004010/004010A1 as well as the future ANSI 005010. The new medical claim form is more compatible with electronic software billing systems. AdvancedMD supports several Bill Types for the UB04 standard form.
