Saving Private Practice Video Script

It wasn’t all too long ago that healthcare was much simpler. When someone got hurt, the doctor was quick to respond, the patient treated, and the only real worry was how to get paid...three chickens or a bushel of corn. Healthcare was a priority...not a business. And the care...much more personal.

So where did all the distractions creep in that keep the independent physician from fully focusing on patient care? And why are today’s physicians overwhelmed with business clutter to the point that they are abandoning the private practice for hospital employment? Frankly, just starting a practice is daunting, not to mention managing it. Hiring and retaining the right people who are capable of handling the complexity of a busy office is critical. And to a small practice owner, business processes such as payroll, HR, taxes, and 401k are all necessary operational functions that add to the stress of running an independent practice and create a drain on the physician’s time.

In addition to the operational aspects that can result in staggering fines for incorrect tax fillings, paper-based practices are faced with a mountain of clinical and claims processing problems. All that time documenting paper charts, searching for documents, faxing and couriering from location to location! Don’t forget about the never-ending calls to confirm appointments and coverage, and phone tag with patients and pharmacies. As your practice grows, how do you expand and manage a multi-site clinic, or hire when you are held hostage by paper? When referring a patient, requesting labs, x-rays, or coordinating a hospital visit, the manual documentation can be a nightmare, not to mention a malpractice risk if you get it wrong.

What about getting paid?...It should be pretty straightforward. You do the work, you should get paid. Right? Wrong! It’s much more technical than you’d ever imagine. Miss insurance verification at the scheduling phase and chalk up another pro bono tax write-off. Loose the super bill before it reaches your billing staff? Your 90-day countdown has started! Forget to log a procedure? Rejection! Billing staff incorrectly codes a claim? Start over!

Whew! Making sure you receive full payments from a variety of sources is tricky business even for the most seasoned billing professional and collecting from patients can be just as tough as from the insurance giants.

Traditional practice management and health records software were designed to be run as separate systems thus creating a host of redundant workflow...Logging into multiple systems, copying and pasting, double data entry, on and on. And then the mistakes from manual processes – more risk, more time, more frustration. Are you starting to get the picture?

If you use an outdated claim checker, without the latest codes, or none at all, you’re dropping loads of money on the floor...30% of all claims are rejected when first submitted – if you are average! Only 50% of all denials are ever resubmitted, an expensive process that requires specialized staff and lots of wasted time for a very small payback, and after the physician hears the bad news on how much work she did without being paid, the fine comes in from the IRS because the manual payroll process resulted in a bunch of filling mistakes – so much for holiday bonuses this year.

How about managing the servers, software, and upgrades? It’s either you or a hired technician? This isn’t what a doctor went to school for. And now that the government is changing all the billing and clinical documentation rules, you’ve got a major IT nightmare on your hands. Ever tried to replace a server blade with a lobby full of patients? No! You do it on the weekend when you should be with your family. Data backup procedures are almost always manual, and often illegal, which adds HIPAA risk. Plus, most data security breaches in a practice are a result of lost or stolen hardware containing patient records. And when something goes wrong, and it will, it can mean a complete shutdown of your practice. IT upgrades are always an added cost with an in-house server...and both hardware and software are subject to theft or even environmental loss – ever see the photos of medical offices under water or leveled by one of the recent tornadoes?

And for the doctor who’s fronting the money to pay for all of this? It may mean a choice of making a house payment, making payroll, a business loan or repurchasing the needed server hardware to keep the business afloat – hard to believe, but physicians are actually going bankrupt these days. A financial balancing act that puts unneeded stress on the doctor and the staff, especially when the practice’s workload is growing heavier.

Regulatory compliance is complex in a clinical setting. Critical training is often overlooked or deemed a costly luxury during practice hours, resulting in staff training during their personal time and often off-site, which can mean they lack detailed compliance knowledge including important HIPAA regulations. This creates added regulatory pressure especially when it comes to audits. All of these areas, although necessary in a medical practice today, simply just weigh down practice, doctors and staff and ultimately impact patient care.

Creating an efficient and effective communications solution to tie all of these areas together can be a distraction and a real hairball. Simply yelling down the hallway doesn’t work any longer. What about accessing your patient information remotely or offering a support team for after hours or non-emergency issues? It doesn’t take long to see why the independent physician practice is about to make the endangered species list.
