AdvancedMD check-in kiosk and patient forms

Patient Relationship Management (PRM) is a new portfolio of products available for AdvancedMD EHR customers. These new tools provide enterprise-level patient communication and population management that are easy to access and use for your practice.

Our new PRM suite gives our EHR customers support for user-customizable patient forms and consent forms, allowing multiple workflow options for your practice. For example, you can assign intake forms and consent forms to the patient prior to their arrival at your office that they complete in the patient portal.

If a patient arrives at your office without having completed all forms, your front office staff can issue an iPad and ask the patient to complete the intake and consent forms prior to seeing the physician. These forms automatically load into the patient chart electronically (no scan required).

Check-in & Admin kiosk (iOS & desktop)


The AdvancedMD mobile patient and administrative kiosk apps work together to save you time in processing patient check-in and managing forms in the patient’s chart.

Your practice’s staff can use the admin kiosk to enter patient information, select documents for a patient, and assign the patient to a patient kiosk device such as an iPad. The admin kiosk also allows you to set up patient kiosk devices.

Assign patient forms (Web admin)


Selection of multiple forms ensures that the patient will get all of the necessary patient forms to fill out for the given scheduled visit. You can manage the entire suite of forms assigned and sent to the patient. You can also remove or retract a form that has been sent, but is no longer required.

Patient Check-in kiosk


Patients can securely read, edit, and sign personal information and documents that are automatically added to their chart. Patients can sign into any mobile device that has the AdvancedMD patient kiosk app installed, even the same device that the admin kiosk app is installed on. The patient kiosk device will not allow a patient to log in until forms have been assigned.

Portal homepage


Portal forms can be accessed from the homepage of the portal. The Patient Forms icon lets you see which family members have outstanding patient forms assigned.

The portal user can determine which patient or family member has the patient form completed. Click the line item to access the form.

Review patient forms (web admin)


You can preview forms that have been submitted by the patient/responsible party to accept, reject, or remove. Accepting the intake and consent forms brings the content of the form into AdvancedMD EHR as a chart document. Rejecting the intake or consent form will send the form back to the patient in either the Kiosk or Portal applications.
