Note Templates for Internal Medicine

Visit – Cardiac – New

This template has fields for assessing a patient’s cardiac symptoms during their initial visit. It includes fields for medical history, surgical history, and family history to assess a patient’s cardiac risk factors. The fields are defaulted to normal settings to allow for charting by exception. Additional template fields include the ability to select the appropriate E&M code from the template, which will send the selected charge to the PM charge slip or EHR prefilled charge slip.

Visit – Cardiac – Established

This template has fields for assessing a patient’s changes since their prior visit, including weight, medication, diet changes. Additional template fields include the ability to select the appropriate E&M code from the template, which will send the selected charge to the PM charge slip or EHR prefilled charge slip.

All exam fields auto-fill from the Visit – Cardiac – New if this template was used during the patient’s initial cardiac visit.

Visit – Headache – New

This template has fields for assessing a patient’s headache symptoms. It includes physical exam fields to provide documentation of a neurological exam. The fields are defaulted to normal settings to allow for charting by exception. Additional template fields include the ability to select the appropriate E&M code from the template, which will send the selected charge to the PM charge slip or EHR prefilled charge slip.

This template has fields for assessing a patient’s changes to their headache symptoms since their prior visit. Additional template fields include the ability to select the appropriate E&M code from the template which will send the selected charge to the PM charge slip or EHR prefilled charge slip.

All exam fields will auto-fill from the template Visit – Headache – New if this template was used during the patient’s initial headache visit.

Subnotes for Internal Medicine

The following subnotes contain fields for the specific conditions. These subnotes can be added to existing or new notes as needed. Subnotes can also be used to build a single patient form to use with patients who have co-morbidities.
