How to Improve Your Digital Patient Intake Process

While many practices have moved intake forms online, front office staff continue to struggle with a simple way to assess one critical question: Is this patient ready for the visit? Digital patient intake speeds up collection, but it does not always show readiness in one place.

Intake also spans more than one channel. In a 2025 survey published in Health Affairs Scholar, 72% of U.S. adults had scheduled at least one appointment by phone in the past year, meaning staff have to consolidate patient information from several inputs.

Following a three-step framework like Know, Act, Standardize helps your front desk team make the complete picture of patient readiness visible, then lets staff act on it before the patient arrives.

Recognize the challenges of ‘digital’ intake

Intake readiness means a patient’s insurance, forms, authorizations, portal access and payment are settled before the patient arrives. When readiness is unclear, gaps tend to surface at check-in when there is the least time to resolve them. Common issues include:

  • Incomplete forms discovered at arrival
  • Insurance cards missing or not yet reviewed
  • Consents that lapsed since the last visit
  • Copays and balances left unresolved
  • Required authorizations missing or expired

The root cause is usually not the forms themselves. Readiness information is spread across separate places: insurance records, forms queues, the patient portal, billing balances and referral records. Each piece may be accurate, but no single view aggregates this information for a quick assessment.

Repeat that across a full schedule, and the front desk workflow becomes a series of lookups, one patient at a time. Results depend on who does the work. Faster collection helps, but for many practices, the larger gain comes from seeing readiness across the whole schedule at once.

Organize intake around Know, Act, Standardize

A practical way to close that gap is to organize intake around three steps:

  • Know: See each patient’s readiness status on the schedule. Staff should not need to open a chart or switch modules to find it.
  • Act: Resolve gaps in the same place your team finds them, in bulk where possible. A missing form or an unpaid copay should be fixable from the schedule.
  • Standardize: Use shared status logic so any staff member follows the same process, regardless of who is working the desk.

Visibility without action creates a longer to-do list. Action without standards leads to uneven results. Together, the three steps turn digital patient intake into a repeatable routine.

In addition, this list of seven pre-visit checks helps your team ensure their work is comprehensive, finding all potential ‘gotchas’ before the patient arrives.

Run seven pre-visit checks before patients arrive

Use the seven checks below as a pre-visit checklist. Each one describes what good looks like and how AdvancedMD supports it. Our Front Desk Command Center lets front desk staff take action on patient appointments and intake directly from the Provider List View without navigating to other screens.

1. Confirm insurance is on file and reviewed

What good looks like: Each patient has current insurance on file, and any card the patient uploaded has been reviewed before the visit.

How we support it: An Insurance column uses color-coded icons. Active insurance cards will display a green icon that can be clicked to open the side panel for more information. A “no insurance on file” icon lets staff scan or import a card. A count of uploaded cards pending review opens Insurance Card Review, where staff can Accept or Decline each card. AI optical character recognition (OCR), which reads text from an image, extracts key fields from card photos and uploads. After staff review the data and Save, the icon updates immediately, without a manual page refresh.

2. Verify required forms are complete

What good looks like: Required intake, consent and demographic forms are complete and reviewed.

How we support it: A red icon means one or more forms are not started, in progress or rejected. Yellow means a submitted form needs staff review. Green means all required forms are complete. Clicking a red or green icon opens Forms Review filtered to the selected date, and yellow opens the first form awaiting review.

3. Validate referrals and authorizations

What good looks like: Any referral or authorization the visit requires is on file and valid for the date of service.

How we support it: Badges show the number of referrals and authorizations for each appointment. Clicking a badge opens the Referrals screen and the Referrals card in the side pane. Catching missing or expired authorizations before the visit can help reduce cancellations and claim denials.

4. Check financial readiness

What good looks like: Staff know the copay, any outstanding balance and whether a card is on file before the patient arrives.

How we support it: Staff can see copay, outstanding balance, credit card on file (CCOF) and payment plan status without switching to the Billing module. They can collect copays as well as capture card details. A separate column tracks if there is a valid CCOF for the patient.

5. Close portal enrollment gaps

What good looks like: Patients have active portal accounts before the visit. Portal access connects them to the self-service side of digital patient intake, including forms, payments and secure messaging.

How we support it: Six status icons show account state. They cover activated, timed out, disabled and locked-out accounts, plus patients with no account, with or without an email sent. Clicking any icon opens Patient Portal Setup.

6. Surface notes that matter

What good looks like: Staff see important patient notes before the visit without opening each appointment.

How we supports i: A Memo icon and a Comments icon next to the patient name show which appointments have notes. The Memo icon appears in both practice management (PM) and electronic health records (EHR), and it updates in real time as memos are created or deleted.

7. Track appointment status and cancellations

What good looks like: Staff can see each appointment’s status quickly and understand why patients cancel.

How we support it: One consolidated status column replaces six separate columns. It shows Confirmed, Checked in, Checked out, Other, Canceled and No-show. Patient-submitted cancellation reasons appear in the same view, so staff can spot patterns and fill gaps sooner.

Standardize the process so it survives turnover

Status icons show what needs attention. A routine makes sure someone acts on it. To keep digital patient intake consistent, consider these practices:

  • Set a ready-by time, such as close of business the day before the visit.
  • Assign an owner to each process check, such as insurance, forms or payments.
  • Review the readiness columns during the morning huddle.
  • Define escalation steps for gaps that cannot be resolved before arrival.

Shared status logic supports this routine. When the same icons and colors mean the same thing to everyone, a new hire reads the schedule the same way a veteran does. Training can focus on what each status means, not where to find it.

For practice managers, the result is a more consistent digital patient intake process across staff and shifts. Write down your routine, owners and escalation steps so the process holds up as the team changes.

Evaluate patient intake software against your workflow

When comparing patient intake software, focus on how well it supports Know, Act, Standardize in your daily front desk workflow.

  • Built into the scheduler, with no extra login or separate integration project
  • Status that updates in real time or near real time
  • The ability to act on a gap from the same screen
  • One shared view of readiness for the whole team
  • A low-risk adoption path, such as an option to switch back to the previous view

Questions to ask vendors:

  • Which readiness items appear on the schedule, and which require another screen?
  • How quickly does status update after a patient submits a form or uploads a card?
  • Which actions can staff complete in bulk?
  • Which add-ons or system versions does each feature require?

To open the Front Desk Command Center, go to Scheduler Center, select the Provider List View tab and click Try Command Center. To return to the previous view, click Legacy Provider List View.

The Front Desk Command Center requires the enhanced scheduler (Scheduler v2). The Forms, Payments and Telehealth columns depend on Advanced Patient, AdvancedMD Pay and Telehealth, respectively. Without those products, the related columns appear locked, with a path to add them.

Make readiness visible before arrival

Digital forms are a strong starting point, but they are one part of intake. The improvement comes from seeing each patient’s readiness before arrival and resolving gaps while there is still time. A clear framework, a consistent checklist and status where staff already work can make digital patient intake more predictable.

If you’re an existing AdvancedMD customer, you can try the Front Desk Command Center by going to the Scheduler Center > Enhanced Scheduler > Front Desk Command Center. The Forms, Payments and Telehealth columns depend on Advanced Patient, AdvancedMD Pay and Telehealth, respectively. If you’re not yet a customer, we’d love to chat.
