Medify Health partners with your practice to provide chronic care management (CCM) and remote monitoring services (RPM) to your eligible Medicare population. Our Health Coaches engage with each patient on behalf of their physician each month to ensure that their healthcare needs are consistently being addressed. Our services allow for care to be provided to the patient in between office visits, which improves the relationship with your practice while improving health outcomes for the patient. The Medify team will ensure that the patients are being scheduled with the practice for annual wellness visits, prescription refill appointments, as well as any issues that may arise requiring an in-office visit.
By deploying cellularly connected monitoring devices to eligible patients, Medify records daily biometric readings from each patient. The biometric readings are then provided to the physician in the patient’s chart in your EMR. The Medify Team is on call seven days a week to monitor the dashboard for any alerts that may show on a patient’s device and respond according to the agreed upon protocols in place between Medify and your practice.
The results for the practice will be an increase in patient compliance, improved patient engagement by eliminating the gap in care between office visits, and an increase in billable services by utilizing Medify as an extension of your staff. There is no financial requirement for the practice to engage with Medify and your patients will appreciate the benefits of having a health coach to meet with each month.
