Upon enrolling in a preventative services program, the patient is assigned a chronic care coordinator and clinical team to help develop a complete care plan based on patient conditions and goals.

This technology offers users periodic “check-ins” from their care coordination team and from the company’s mobile/smartphone-enabled virtual care assistant, also known as “VAL” (Virtual Assisted Living). At the same time, physicians get regular updates and alerts, thereby enabling them to review care plans, identify and proactively address risk, and if needed, trigger live engagement between care team members, providers, and appropriate community resources.

Cosán also offers robust reporting dashboards that provide real-time data on key performance indicators and time-tracks every care management activity, creating an audit trail that meets Medicare’s chronic care management requirements.

Behavioral Health Integration

In addition to the solutions described above, Cosán’s services include a comprehensive behavioral health integration (BHI) platform that supports individuals with behavioral health conditions, developmental disabilities, and drug addiction. The company also provides quality payment program support, advanced care planning and medication management solutions, scheduling/call center support technology, and a complete clinical staffing solution.
