Use this template to document the assessment of a new patient with a diagnosis of allergies or asthma. The template includes fields to document medical and family history specific to allergies and asthma. Additionally, you can document environmental history, allergy symptoms, an asthma control questionnaire, and a lung function test. - Visit – Allergy/Asthma – Established
Use this template to document the ongoing assessment of an established patient with a diagnosis of allergies or asthma. The template includes fields for documenting a patient’s history since their last visit and the ability to document a lung function test. - Visit – Opioid – New
Use this template to document the assessment of a new patient’s psychiatric history, opioid history, substance use, opioid use, and withdrawal symptoms. The template includes fields to document an HPI, physical exam, test results, and treatment goals. - Visit – Opioid – Established
Use this template to document the ongoing assessment of an established patient with a history of opioid use. The template includes fields to evaluate current opioid use, withdrawal symptoms, physical exam, and treatment goals.

Use this patient form to document a patient’s medical, surgical, and family history. The form includes fields to document a patient’s initial environmental history, allergen symptoms, review of systems, medication allergies, and an asthma control questionnaire. If the patient form is filled out prior to the visit and saved to the patient’s chart, it will auto-fill if the User Preference is set to “Fill from last field values.” - Visit – Allergy/Asthma Hx – Established
Use this form to document the ongoing assessment of an established allergy or asthma patient. The form also includes an asthma control questionnaire. If the patient form is filled out prior to the visit and saved to the patient’s chart, it will auto-fill if the User Preference is set to “Fill from last field values.” - Visit – Opioid Hx – New
Use this patient form to document a patient’s psychiatric history, opioid history, substance use, opioid use, and withdrawal symptoms. This form also includes fields to document a social, medical, and family history. If the patient form is filled out prior to the visit and saved to the patient’s chart, it will auto-fill if the User Preference is set to “Fill from last field values.” - Visit – Opioid Hx – Established
