E/M Subnote Templates

The E/M subnotes are designed to be dropped into an existing patient note to help you learn about the new 2021 E/M level of service guidelines for Office and Other Outpatient Encounter Codes. (99201-99215)

Each of the E/M subnote templates has a word merge document attached that provides additional details about the 2021 E/M changes and the CPT coding definitions.

The subnotes are located in the EHR Template library General folder but when you download the subnotes they display in a folder named _E/M for 2021.
